About this test

ESR (Erythrocyte Sedimentation Rate) measures how fast red blood cells settle at the bottom of a tube in one hour. When there is inflammation in the body, proteins in the blood make red cells clump and fall faster, so ESR rises. It is a simple marker of inflammation, not a test for one disease.

Why is this test done?

  • Check for inflammation in the body
  • Help find the cause of long-lasting fever
  • Monitor arthritis, TB and other inflammatory diseases
  • Often done together with CBC

Symptoms — when should you get tested?

  • Fever lasting many days
  • Joint pain and stiffness
  • Unexplained weight loss
  • Night sweats
  • Headache with scalp tenderness (in older people)

Normal range

Men 0 – 15 mm/hr
Women 0 – 20 mm/hr
Above 50 years slightly higher values are normal

Reference ranges differ slightly between labs, methods, age and sex. Always discuss your report with your doctor.

If your result is high

A high ESR means there is inflammation somewhere in the body. Your doctor will use other tests to find where.

Common reasons
  • Infections, including tuberculosis
  • Rheumatoid arthritis and autoimmune diseases
  • Anaemia
  • Pregnancy and older age
  • Kidney disease or some cancers

If your result is low

A low ESR is usually not a concern.

Common reasons
  • Polycythaemia (too many red cells)
  • Sickle cell disease
  • Very high white cell count
