About this test

The Thyroid Profile measures T3, T4 and TSH. The thyroid gland in your neck makes T3 and T4, which control your metabolism, energy, weight and heart rate. TSH comes from the brain and tells the thyroid how much hormone to make. Together they show whether your thyroid is underactive (hypothyroidism) or overactive (hyperthyroidism).

Why is this test done?

  • Check thyroid function
  • Find hypothyroidism or hyperthyroidism
  • Monitor thyroid medicine dose
  • Screening in pregnancy and for women planning pregnancy

Symptoms — when should you get tested?

  • Tiredness and feeling cold
  • Weight gain or unexplained weight loss
  • Hair fall and dry skin
  • Fast heartbeat, anxiety or tremors
  • Irregular periods or difficulty conceiving
  • Swelling in the neck (goitre)

Normal range

T3 (total) 0.8 – 2.0 ng/mL
T4 (total) 5.1 – 14.1 µg/dL
TSH 0.4 – 4.2 µIU/mL

Reference ranges differ slightly between labs, methods, age and sex. Always discuss your report with your doctor.

If your result is high

A high TSH with low T4 usually means an underactive thyroid. High T3/T4 with low TSH means an overactive thyroid.

Common reasons
  • Hashimoto's thyroiditis (high TSH)
  • Graves' disease (high T3/T4)
  • Iodine imbalance
  • Thyroid medicine dose too low or too high

If your result is low

A low TSH often means the thyroid is overactive; low T3/T4 means it is underactive.

Common reasons
  • Hyperthyroidism (low TSH)
  • Hypothyroidism (low T4)
  • Pituitary problems (rare)
  • Severe illness
