About this test

C-peptide is released in equal amounts with insulin from the pancreas. Measuring it fasting and after a meal shows how much insulin your own body is making, even if you take insulin injections.

Why is this test done?

  • Tell type 1 from type 2 diabetes
  • Check how much insulin the pancreas still makes
  • Investigate low blood sugar episodes
  • Help decide if insulin is needed

Symptoms — when should you get tested?

  • Newly found diabetes in a young person
  • Frequent low sugar episodes
  • Poor sugar control on tablets

Normal range

Fasting 0.8 – 3.1 ng/mL
Post-meal (2 hours) 2.0 – 8.0 ng/mL

Reference ranges differ slightly between labs, methods, age and sex. Always discuss your report with your doctor.

If your result is high

High C-peptide means the pancreas is making a lot of insulin.

Common reasons
  • Insulin resistance and type 2 diabetes
  • Insulinoma (insulin-making tumour)
  • Kidney disease
  • Some diabetes tablets (sulphonylureas)

If your result is low

Low C-peptide means the pancreas is making little insulin.

Common reasons
  • Type 1 diabetes
  • Long-standing type 2 diabetes
  • Damage to the pancreas
